it worked on my phone. click on the link underneath the picture. on my computer it just seems like a bunch of jumbled mess.

yes, click right under the product picture, where it has the words replace with btn img-hide span class req-btn then the address form will come up.:)

They no longer have it on the computer site, but it is still on this app. I typed in free sample in the search, clicked under the pic, but nothing. When I tried it on the phone, I clearly saw, "Request A Sample" but there was a small section left off where I guess you click submit or whatever. The form and "Request A Sample" is still up as of right now, but not on the site from my computer. Weird!!!
