When you click on the button "Get Yours Now" you may find that the request form is for the Healthcare Scoop Digital Thermometer. I wouldn't worry about, I think they just forgot to change the form from the previous offer.

It worked for me. They want you to sign up with them and then you have to read this long Terms. We'll see.
