You have to fill out the first part of the "request information", then u will fill out the rest, after u choose "Home" or "business" then u fill out ur address and the select Free sample at the bottom, and mine went thru

This is not free, it costs $1.00! Even if you are able to submit the form, you'll get an email from them saying they need you pay $1.00 before they'll send the sample.
